With 90-minute initial visits, we make joining a new practice easy and ensure a smooth transition of care.

Prior to the appointment, we confirm insurance and request a transfer of all medical records to ensure our team is up to speed by the time you arrive.

• Upon arrival, our intake team will welcome you to the practice and assess vitals and current medications.
• Our board-certified provider will have an in-depth conversation about your medical history, primary health concerns, and long-term goals.

Personalized Care Plan
• Our provider will partner with you to develop a customized care plan based on your discussion.
• Before leaving, our team will order any necessary labs, assist with referrals, and schedule a follow-up visit.
