We’re not your typical Primary Care practice—here’s how we’re different:
We’ll listen to your concerns and work with you to create a care plan that feels just right for you.
We take the time to listen and include you in decisions about your health, so you feel confident in your care every step of the way.
We focus on proactive, preventive care when you need it, to help you stay healthy and address potential issues early.
Your first visit is focused on building a strong foundation for your care and will include:
If you are a new Medicare patient, you will meet with an Insurance Support Specialist who will introduce you to the benefits of our practice and how we’ll support your health.
A Medical Assistant will check your vitals, go over your medications, and learn about your health priorities.
During your visit, you’ll have dedicated time with your new provider to get to know each other, discuss your health history, concerns, and goals, and work together to create a care plan tailored to your needs.

Note: Your provider will perform a brief physical to assess your health and establish care, but this visit will not include a full physical or Annual Wellness Visit, so you won’t need to change into a gown. There’s no need to fast ahead of time. If fasting labs are needed, we’ll schedule those separately.
To help us provide the best care, please bring:
• Your insurance card
• A photo ID
• A list of any medications, vitamins, and supplements you’re taking, including dosages.
We’ll send you a link to your new patient forms 7 days before your visit.
Filling them out early helps us get your information right, like your contact details, insurance, pharmacy, and health history. This way, your provider can spend more time focusing on you and your care during your visit!
Your visit will be billed as a standard office visit, and any copay listed on your insurance card will be due at the time of your appointment.
