Here’s What to Expect at Your First Visit

Your first visit is focused on building a strong foundation for your care. It will last about 90 minutes and include:

    Meeting Your Team

    A Patient Service Liaison will introduce you to the benefits of our practice and how we’ll support your health.

  • 02

    Personalized Health Review

    A Medical Assistant will check your vitals, go over your medications, and learn about your health priorities.

  • 03

    Time With Your Provider

    During your visit, you’ll have dedicated time with your new provider to get to know each other, discuss your health history, concerns, and goals, and work together to create a care plan tailored to your needs.


Herself Health Appointment Room

Note: Your provider will perform a brief physical to assess your health and establish care, but this visit will not include a full physical or Annual Wellness Visit, so you won’t need to change into a gown. There’s no need to fast ahead of time. If fasting labs are needed, we’ll schedule those separately.

Preparing for Your Visit

    What to Bring

    To help us provide the best care, please bring:

    • Your insurance card

    • A photo ID

    • A list of any medications, vitamins, and supplements you’re taking, including dosages.

  • Paperwork icon

    New Patient Forms

    We’ll send you a link to your new patient forms 7 days before your visit.

    Filling them out early helps us get your information right, like your contact details, insurance, pharmacy, and health history. This way, your provider can spend more time focusing on you and your care during your visit!

  • Insurance icon

    Billing Information

    Your visit will be billed as a standard office visit, and any copay listed on your insurance card will be due at the time of your appointment.
